Bicycle Test

Purpose: The bicycle test (of VanGelderen) is used to differentially diagnose between neurogenic claudication and vascular claudication.
Procedure:
Positive Result (for Neurogenic Claudication): Pain is relieved upon bending forward and worsened when bending backward. This is a sign of neural compression (spinal stenosis). Positive Result (for Vascular Claudication): Pain remains, and increases as the cycling continues. This is a sign of arterial involvement or limb ischemia that requires further testing. |
