Wells Criteria
Purpose: Detect the likelihood of deep venous thrombosis (DVT).
Procedure: Use the following criteria to score your patient on their risk for deep venous thrombosis. Add up the scores for each question,
Key
Medical consultation advised in the presence of low probability, (treat and refer)
Medical referral required with moderate or high score
Normal Result: No to low probability of DVT.
Abnormal Result: High probability of DVT.

Procedure: Use the following criteria to score your patient on their risk for deep venous thrombosis. Add up the scores for each question,
- Active cancer (within 6 months of diagnosis or receiving palliative care) (score 1 or 0)
- Paralysis, paresis, or recent immobilization of lower extremity (score 1 or 0)
- Bedridden for more than 3 days or major surgery in the last 4 weeks (score 1 or 0)
- Localized tenderness in the center of the posterior calf, the popliteal space, or along the femoral vein in the anterior thigh/groin (score 1 or 0)
- Calf swelling >3 cm compared to the other leg--Measured 10cm below tibial tuberosity(score 1 or 0)
- Collateral (nonvaricose) superficial veins present (score 1 or 0)
- Pitting edema, confined to symptomatic leg (score 1 or 0)
- Previously documented DVT (score 1 or 0)
- Alternative diagnosis to DVT as likely or more likely (score 0--no or -2 yes)
Key
- -2 to 0 Low probability of DVT 3% (95% confidence interval [CI] 1.7%–5.9%)
- 1 to 2 Moderate probability of DVT 17% (95% confidence interval [CI] 12%–23%)
- 3 or more High probability of DVT 75% (95% confidence interval [CI] 63%–84%)

Normal Result: No to low probability of DVT.
Abnormal Result: High probability of DVT.
